Clinical placement log book
Whilst on placement, students must keep a log of all activities:
The log should be discussed with the field supervisor and emailed fortnightly to the university supervisor to be vetted to ensure that the goals and terms of the practicum are being met. Each page of your log should be initialled by yourself and your field supervisor. A final hard copy of the log must be signed by yourself and the field supervisor at the end of the placement (see template for layout of final signature page) and submitted with your evaluation forms for signature by your university supervisor.
