Clinical placement completion requirements

Mid Placement Review

Once approximately half the placement has been completed, an interim review should be conducted which involves the student and field supervisor reviewing progress, the student’s performance, and the extent to which the placement objectives are being met. A copy of this review should be sent to the placement administrator and any action that is required as a result should be taken.

In some instances it may be advisable to hold a mid-placement meeting involving the student, field supervisor and university supervisor

End of placement meeting and evaluation

Students should organize a final review meeting with their field supervis or on completion of each placement. At this meeting, students should receive and present feedback. Evaluation forms must be completed at or prior to this meeting.

Three forms must be completed:

• Short placement Evaluation Form (doc) or Long placement Evaluation Form (doc)
  To be completed by the Field Supervisor and the original returned to the Psychology Programs Administrator, Krongold Centre, Room G08.
• Field Supervisor Feedback Form (doc)
  To be completed by you and returned to the Psychology Programs Administrator, Krongold Centre, Room G08.
• Placement Report Form P3 (pdf)
  To be completed by your Field Supervisor and retained by you. This form will be required when you apply for full registration with the Psychologists Registration Board. A copy should be submitted to Psychology Programs Administrator, Krongold Centre, Room G08.

The Evaluation form and P3 form must clearly state the number of full (8hour) days completed and must tally exactly with your log book for each placement.
