Clinical placement completion requirements

 

Mid Placement Review

Once approximately half the placement has been completed, an interim review should be conducted which involves the student and field supervisor reviewing progress, the student’s performance, and the extent to which the placement objectives are being met. A copy of this review should be sent to the placement administrator and any action that is required as a result should be taken.

In some instances it may be advisable to hold a mid-placement meeting involving the student, field supervisor and university supervisor

End of placement meeting and evaluation

Students should organize a final review meeting with their field supervis or on completion of each placement. At this meeting, students should receive and present feedback. Evaluation forms must be completed at or prior to this meeting.

Two forms must be completed:

The Evaluation form must clearly state the number of equivalent full (8hour) days completed and must tally exactly with your log book for each placement.

End of Placement Summary Form

On completing all three placements students should also complete and submit a Placement Summary Form. Placement Summary Form (doc).

This form summarises all hours, client contact hours and equivalent full time days completed across all placement settings.  The final result for the last placement completed cannot be processed until all forms are submitted.
